DOT physicals are comprehensive medical examinations required by the Department of Transportation. It is for individuals seeking to obtain or renew a commercial driver's license. This service is included in our membership; however, membership is not required.

Click here to download and fill out page 1 and 2 of the Medical Examiner Report Form ahead of time. Fill out page 1 and 2 only and put your name and date of birth at the top of pages 2, 3 and 4. Leave the exam date blank. Bring all pages with you.

Cost:

Members: One free DOT Exam per year with standard or premium membership.

Non-Members or Light Membership: $85 per DOT Exam.
