Services provided by the Physician shall include:
Enrollment in a medical practice with limited Patient enrollment in order to ensure ease of access and personal attention
Direct access to the Practice and the Physician, at all times possible by phone, text or e-mail
Flexible appointment scheduling
Annual exams, routine and sick visits, chronic disease care, medication management during up to 12 visits (telehealth and in-office combined) per year, during regular office hours
Unhurried, quality consultations
Individualized care and guidance with medical decisions
Below is a non-exclusive list of the Practice’s Services. This list is subject to change, as the Practice and the physician change or expand or remove Included Services, and other care options and opportunities for the patients.
BASIC CARE:
Preventative Care & Tests Planning
Well Infant & Child Wellness Exams (including Camp/Sports Physicals; age appropriate up to the age of 2, one per year from age 2 and older)
Wellness Exams for Adults (1 per calendar year)
ACUTE CARE: (Where appropriate for primary care office visit.)
Concussions
Gastrointestinal Problems (e.g. stomach flu, diarrhea, peptic ulcer)
Gynecological Problems
Headaches/Migraines
Injuries (minor injuries, no stitching or casting)
Rashes
Sexually transmitted infections
Upper and Lower Respiratory Infections (e.g. common cold, strep, pink eye, sinusitis, pneumonia)

COMPLEX PRIMARY CARE: (Specialty care not included.)
Cardiovascular (e.g. high blood pressure, cholesterol)
Dermatological (e.g. acne, eczema, other dermatitis)
Endocrine (e.g. hypothyroid, diabetes, obesity, osteoporosis, hormone replacement)
Gastrointestinal (e.g. GERD, ulcers, IBS, constipation)
Genitourinary (e.g. incontinence, erectile dysfunction)

Puncture aspiration of ganglion cyst
Removal of foreign body from ear/nose (removal not guaranteed)
Removal of foreign body from eye, loose, or in cornea only with dye testing for corneal damage (removal not guaranteed)

Trigger finger injections (Limit 1 per 6 months in same finger.)
